1. 2024 Student INFORMATION Form
(Must complete only once per year).

All Stars are required to complete this form BEFORE booking any classes including trial classes.

2. 2024 Term 4 Class BOOKING Form

Options include: Full fee payment, $50 deposit or NDIS funding. No surcharge (DSA will absorb this). 

Note – The ‘Student Information Form’ must be completed prior to booking any class.

3. Pay Class BALANCE

If you have only paid a deposit for your Star’s class/classes, please pay the remaining balance here BEFORE your Star’s first class. 

PLEASE NOTE - Full payment is due prior to your Stars first class.
Please contact Donna BEFORE payment is due to negotiate an alternative payment plan.
Failure to pay full fees prior to first class, will incur a $33 late fee, unless a payment plan is agreed upon in writing prior to first class.
