2. Class BOOKING Form

Options include: Full fee payment, $50 deposit or NDIS funding. No surcharge (DSA will absorb this). 

Note – The ‘Student Enrolment Form’ must be completed prior to booking any class.

3. Pay Class BALANCE

If you have only paid a deposit for your Star’s class/classes, please pay the remaining balance here BEFORE your Star’s first class.
